What To Expect From Us

With this first contact, you may be asked the following questions:

1. What type of insurance do you have? If you do not have insurance that I accept, are you willing to pay “out of pocket” to see me?

2. What are you seeking treatment for? (I do not treat individuals with anorexia or those with severe and persistent mental illness in my private practice. My definition of “severe and persistent” is having psychiatric symptoms severe enough to qualify for disability. This is due to my treatment approach, office work environment, and how my practice is set up as a solo-provider.)

3. Have you reviewed the information on my website? Do you have any questions for me so far?

4. I only have openings at this time on weekdays from 9:30 am to 2:30 pm. Will this schedule work for you? 

Your initial evaluation with me will take place over a 45 – 60 minute session. The first session is scheduled for an hour max.

Following the initial evaluation, we will work together to set treatment goals and make a plan to help you reach those goals. This plan may include medications, supplements, diet changes, behavioral strategies, therapy, or referral to other specialists.

Please bring your driver’s license and insurance card to your first appointment. I will need to make a copy of both of these for my records.

Your first payment will be due before proceeding toward your first session. I often have my clients fill out screeners to monitor progress at the beginning of each session as well.

I ask that you bring or have your doctor/practitioner fax copies of any recent lab tests by your second session. If you are seeing, or have seen, any other providers who have pertinent health information on you, I will ask that you sign a Release of Information “ROI” for each of these practitioners so this information can be obtained and reviewed during the initial assessment process. I will provide you with this form in session, or I can e-mail it to you.
